A dentist–patient relationship is only established after an in-person or telehealth clinical evaluation and mutual agreement for treatment.

6. PAYMENT TERMS
Patients are responsible for:
Payment of services at the time of treatment unless prior arrangements are made.
Deductibles, co-payments, and non-covered services.
Any balance not paid by insurance.
Insurance verification is not a guarantee of payment. Final coverage determination is made by your insurance carrier.
Failure to pay balances may result in referral to a collection agency where permitted by law.
7. FINANCING & THIRD-PARTY PAYMENT PROVIDERS
We may offer financing through third-party providers. Approval, terms, and conditions are determined solely by the financing provider.
Park Mall Dental Centre is not responsible for financing decisions, terms, or disputes between you and third-party lenders.
8. TELEHEALTH SERVICES
Telehealth services may be offered when appropriate.
By participating in telehealth services, you acknowledge:
Telehealth may have limitations compared to in-person visits.
Technical failures may occur.
You are responsible for ensuring a private and secure environment on your end.
Telehealth services are subject to applicable laws and licensing regulations.
9. SMS TEXT MESSAGING TERMS
By providing your mobile number, you consent to receive SMS messages from Park Mall Dental Centre for:
Appointment reminders.
Treatment follow-ups.
Office updates.
Billing notifications.
Promotional communications where permitted by law.
Message frequency may vary. Message and data rates may apply.
You may opt out at any time by replying STOP. Upon sending STOP, we will confirm your unsubscribe status via SMS. To rejoin, sign up as you did initially and we will resume sending SMS messages to you.
